Looking for a Lost Plot Pamphlet

A couple of Nanos ago, I found online and printed out this several-pages-long sort of plot pamphlet that one could fill out (complete with spaces to draw your own pictures) and refer back to during Nanowrimo. It also had a couple of extra pages in the back with a random plot twist generator and things like that.

I looked around on Google but haven't been able to find it... it wasn't year-specific or anything. Anybody seen it around?

I am not sure if this is what you are looking for. If not it is certainly as helpful. :-)
